Education:

Physical/Mental State at Examination: Feeble condition, middle sized, nervous temperament. Emaciated, nearly toothless, eats very badly, always been a small eater. Bowels open. Tongue clean. Heart's impulse normal. Slight arterial atheroma. Respiratory murmur over apices somewhat loud. A little bronchophony. Was completely incoherent on admission and was full of religious delusions. Said that she had conquered the devil and that she had seen the Holy Ghost. Memory very much impaired and loss of identity present. Says that she is at home with her people and that I am some William Jones, a friend of hers. Has been always more or less weak minded but the mental derangement has taken a more active form only recently. She has green irides with dilated but equal pupils. Has always lived with her sister who could always manage her until recently. Placed in No 4 Ward.

Current Diagnosis: Schizophrenia (F20.1)

Case Notes

1875 May 31 - Placed to sleep last night in associated room when she was pretty quiet though sleepless.

Eats but little, apt to get somewhat excited. Walking about briskly.

Talking about conquering the devil. Haust. Chloral H.S.

June 1 - Slept pretty well.

Very quiet during the day.

Appears quite harmless.

Appetite being bad was ordered bread and milk at 11 am.

Rep. Haust. Chloral.

June 3 - Apt to get excited, walking about and slapping some of the patients, working in the intervals. Quite harmless.

June 18 - Perfectly incoherent and full of delusions.

Complete loss of identity.

Very talkative, very noisy at night, removed to cell bedroom.

Has frequent draughts.

Aug 17 - Just the same. Will never be any better.

Dec 21 - Much improved in health, has been slowly getting better but her delusions remain.
