Physical/Mental State at Examination: Feeble health. Nervous temperament. Greatly emaciated. Looks pale and cadaverous. Refuses food except upon compulsion. Bowels costive. Emphysematous. His insanity has been coming on for the last 12 months gradually but has only assumed an acute form within the last 12 weeks. Since then he has become silent, cross and taciturn. His insanity was probably a cause and not a result of his giving up his situation. Would only mumble incoherently in reply to a question. Took his food with difficulty. Ordered Haust. Chloral HS. Ordered beef tea and bread and milk.

Current Diagnosis: Severe Depressive Episode (F32.3)

1875 Oct 1 - Has not spoken.

Struggled violently when he was being fed today. Had some sleep last night.

Oct 2 - Keeps the same.

Is obliged to be fed in opposition to his will. Oct 9 - Still very desponding. Does not speak unless spoken to.

Takes his food very much better.

Sleeps very much better.

Nov 1 - No change.

Medications/Treatments: Chloral. Fed by force.
