I have known this patient to be mentally affected for years but recently she has got much worse, very dejected and full of delusions, she sees things about her, sometimes cattle, sometimes like fire, is exceedingly restless and sleepless. Her sister Harriet Williams of Bronllan Llysfaen with whom she has lived for 2 years, says that she has got much worse lately, very restless, dejected and sleeping but little at night, being up and down and requiring someone to look after her continually. Physical/Mental State at Examination: Health moderate. Patient was for years in service. Has for about 12 years lived with her sister in the Llysfaen Post Office. She has been strange for a long time and during the past 3 years obviously so. No cause is given.

Current Diagnosis: Depressive Episode (F32.3)

Case Notes

1905 July 1 - Average height and build. Close cropped brown hair, grey irides, pupils normal size but sluggish. She is apparently quite blind but does not acknowledge it and says she can see quite well. She is rather depressed and says she wishes she were dead and wishes me to give her poison. She believes she is surrounded by floating objects which she cannot describe, illusions. She is quiet and orderly and gives no trouble.

7 - Shouts and screams at times that "things" surround her.

Hallucinations of sight. Eats and sleeps well.

Aug 3 - Troublesome. Excited and tries to get thro the window. Clings to everyone. Wets herself.

Oct 1 - Down in 7. Very noisy and excited at times. Dec 3 - The same. 1906 Feb 1 - No change.

June 15 - Very troublesome and has to have her own way. Lies about on floor, under tables and like a dog and is generally only partly dressed.

Same delusions. Moderate health. Sept 8 - No change,

1907 Feb - Not so strong and is possibly Phthisical. Otherwise the same.

1908 Jan - Has been failing in strength for some time and confined to bed. Very dirty and troublesome. Mar 1 - Died today of Exhaustion.
