Physical/Mental State at Examination: Fair health and condition. Bruise on forehead. A young imbecile of low intelligence. Can carry on an elementary conversation. On admission she was constantly trying to kick or punch anyone who came near her.

Current Diagnosis: Learning disability

1920 July 27 (copy) - Suffering from Imbecility.

Very low intelligence. Restless and destructive in her habits.

Has passionate outbursts and tries to injure herself. Very noisy and also dirty in her habits. Requires constant attention.

Aug 3 - Her habits have improved and she is now better behaved and tries to control her pinching and kicking propensities.

12 - Behaving fairly well.

Nov 12 - Unchanged mentally.

1921 Jan 2 - Very mischievous.

Moderate health and condition.

15 - Now confined to bed with cough and pyrexia.

Difficult to make a physical examination of the chest as she will not keep still. Apr 2 - Bad tempered and mischievous at times.

Better health again.

June 24 (copy) - Suffering from Imbecility.

Oct 2 - The same.

1922 Jan to 1923 Oct - Notes indicate no further change.

1924 Jan to 1925 Apr - Unchanged.

July - Suffering from TB Pulm.

Oct 6 - Filthy habits, bad tempered at times and requires constant attention.

The TB is in a state of remission at present. 1926 Jan - Unchanged mentally. Confined to bed. Symptoms of abdominal tuberculosis.

10 - There has been considerable pain and vomiting.

Haematemesis on two occasions. 12 - Died.

PM disclosed TB in both lungs and ulceration of stomach.
