She informed me that she was living in terror of a person named Ann Price who she states is coming to strike her with a poker, also that some person unknown is going to chloroform her. Mrs E J, (neighbour), states that M L W has threatened to strike her, that she tears her clothing and also bedclothes.

Physical/Mental State at Examination: Fair health and condition. She states that certain people have been annoying her and that she can still hear their voices, but cannot state what the voices say. Her conversation is fairly rational on other subjects but her memory is not good.

Current Diagnosis: Delusional disorder (F22)

Case Notes

1920 Sept 18 (copy) - Suffering from delusional insanity.

She has delusions of persecution and states that she is in fear of certain people whom she mentions by name, she states she hears voices speaking to her and cannot quite understand what they mean. She states also that she has at times (been) impulsive which makes her destroy her clothing, especially old clothing.

Her memory is not good and on admission she was inclined to refuse her food, but is now better in that respect.

Oct 18 - Still hears voices and has the same delusions. Well conducted on the whole and is able to play the piano quite well at times.

Nov 15 - Still confused and her memory is defective. Hears voices at times but not as much as before.

Rather childish but is able to employ herself in the workroom.

Dec 18 - No change.

1921 Jan 15 - Dull and indifferent.

Apr 2 - Condition varies. At present quiet and indifferent.

Does not employ herself.

July 1 - Unimproved mentally.

Aug 16 (copy) - Suffering from delusional insanity. She has delusions of persecution and states that certain people of whom she is in great fear persecute her.

Has delusions about the appearance of her face and states that it changes its shape every day. Curt and abrupt in manner.

1922 Jan - Still deluded. Varying moods but will employ herself at intervals. Good at the piano.

Recently broke a window.

Apr - No change.

June - Suffering from an abscess of the right foot, a large amount of pus was evacuated.

July 2 - Part of the skin sloughed, leaving a foul ulcer, this is now healing over.

She has now another abscess forming behind the left knee.

Mentally she has not changed at all.

20 - The abscess is enlarging and burrowing in all directions.

A large amount of foul pus evacuated.

Becoming very feeble.

16 - Died.
