Medical Certificate:
Refuses to take any nourishment or to attend to his personal cleanliness and habits. Refuses to answer questions and complains of 'seeing devils'. His father J T, Mynytho, nr Pwllheli, farmer, says that his son W has been in bed for over 4 yrs. He took to his bed for fear of being conscripted for the Army. Has been quiet and easily managed until last Sunday when he suddenly thought his sisters were trying to poison him and has refused food since. Education:

Physical/Mental State at Examination: Fair health and condition. Fell on ice when 7 yrs old and remained unconscious for half an hour. On admission is very dull and stupid and rather depressed. Very lazy and will not work. Suffering from Nephritis.

Current Diagnosis: Learning disability

1920 Aug 5 (copy) - Suffering from Imbecility and Melancholia.

He is a man of low education and says that he passed to Standard I. He is depressed and apprehensive and states that he has been afraid of people trying to poison him.

He is inclined to be untidy and dirty in his habits.

11, 18, 25 - The same.

Sept 5 - Suffering from Oedema of both feet. Kidneys both palpable and large.

Oct 1 - No change.

Nov 10 - Much better physically now and gets about.

No improvement mentally. 1921 Jan - No change. Apr - Losing weight of late.

No mental improvement.

July 5 (copy) - Suffering from Imbecility.

He is a man of low intelligence who is depressed and takes no interest in his surroundings.

Keeps himself apart from all the others.

Jul to April 1922 - Notes indicate no further change.

July (copy) - Suffering from Dementia. Memory is defective. He walks about talking to himself and taking no interest in his surroundings. Unable to apply himself to anything useful and latterly has become very untidy in his habits.

Oct to April 1923 - No change.

1924 July (copy) - Suffering from Imbecility and also some degree of Melancholia.

Very depressed.

Keeps a great deal to himself and is generally found speaking with his head looking to the ground the whole of the day.

Aug - Now in bed suffering from Pulmonary TB which is rapidly progressing.

Sept 16 - He has now become very ill and wasted and has cavities in both lungs. Hectic temp. And night sweats.

Oct 10 - Died today from Pulmonary TB.
